Office location:

My office is located in the red brick building at 4315 Guadalupe Street (on the east side of Guadalupe, just 1 1/2 blocks south of 45th Street). Ample parking is available under the building, as well as on neighboring side streets. To reach my office, take the elevator to the 2nd floor and follow the hallway all the way to the end. My office will be on your right, in suite 208. Upon entering the waiting room, you will see an inner door with two light switches to the right. Press the upper part of the switch with my name on it and have a seat; I will meet you in the waiting area at our scheduled appointment time.

Fees:

$210 per individual or couples session
$70 per week for single-leader process group
$80 per week for co-led process group
$90 per week for co-led training group

Payment by cash or check is due at the time of service. I raise my rates every few years, and I always give clients two months’ notice before doing so. For those with financial need, reduced fee services are available on a limited basis.

I am not on any insurance panels. However, many policies have out-of-network coverage, so your treatment may still be covered by insurance. Since coverage varies widely, I encourage you to contact your insurance company about out-of-network benefits prior to our initial meeting. Please note that if you choose to submit claims to your insurance company, you will still be responsible for the session fee at the time of service.

In the event you need to cancel your individual/couples’ therapy appointment with me, kindly give me at least 24 hours’ notice before the start time of the appointment. If for any reason you are unable to do so, you would be expected to pay for that session.

Forms:

You will need to review and complete three brief forms before our initial meeting. You may download the forms below or, if you prefer, I can provide them for you prior to your first visit. Please don't hesitate to ask any questions about the information covered in these forms when we meet.

The following HIPAA Notice describes how your personal information may be used and/or disclosed. I am required by law to provide this information to you, and your signature on the "Office Policies Form" acknowledges that you have received and understand it.
